Why the study?
The modalities of preoperative ECG prescription, the abnormalities found, and their impact on perioperative management at the Owendo University Hospital were not described.
Cross-Sectional (n=105)
No
Routine preoperative ECG in mostly young, healthy patients has a low yield for major abnormalities and rarely changes perioperative management.
Conclusion. ABSTRACTIntroduction. The electrocardiogram is frequently used to assess a preoperative patient's cardiovascular condition. The aim of this work was to describe the modalities of its prescribing, the abnormalities found and their consequences on perioperative care at The University Hospital of Owendo. Methods. This was a retrospective, descriptive study of the analysis of patient records received in pre-anaesthetic consultations at the Owendo University Hospital Centre between July 1 and December 31, 2020. Results. A total of 105 patients received an electrocardiogram, representing a completion rate of 17.12%. The average age of patients was 45.40 ± 18.84 years and the sex ratio was 0.59. Patients under 65 years of age were the most numerous (83.81%). High blood pressure was the most commonly found classic cardiovascular risk factor (27.61%). Patients were more frequently in ASA 1 classes (55.24%) 2 (39.05%). They had to undergo minor surgery in 59.05 of the cases. Electrocardiogram was abnormal in 30.48% of patients. Minor electrical abnormalities were recorded in 48.27% of hypertensives (p -0.01). The presence of other conventional cardiovascular risk factors was not significantly associated with electrical abnormalities. Only one major electrical anomaly (0.95%) was recovered, justifying a postponement of the surgery for therapeutic adjustment. Conclusion. The cost-effectiveness of the electrocardiogram in preoperative anesthetic safety is low. Far from being systematic, the prescription of the electrocardiogram should be guided by history, physical examination and the type of surgery.
