Key result
Intradialytic food ingestion reduces MAP ~10% via decreased SVR, unaffected by caffeine.
Why the study?
The hemodynamic mechanism of food ingestion-induced hypotension during hemodialysis and the effects of caffeine on this phenomenon were unclear.
RCT (n=10)
Double-blind
Absolute Event Rate: -12.4% vs -2.4%
p-value: p=<0.05

Intradialytic feeding may worsen hypotension during dialysis; leaves open effective countermeasures and requires confirmation in larger trials.
Barakat et al. (1993) conducted an RCT in Hemodialysis (n=10). Food ingestion and caffeine pretreatment vs. Placebo/no meal was evaluated on Percent change in mean arterial pressure at 30 min after food ingestion (p=<0.05). Intradialytic food ingestion significantly decreased mean arterial pressure (-12.4 vs -2.4; P<0.05) due to reduced systemic vascular resistance, which was not attenuated by 200 mg of caffeine.
