Drawing on social exchange theory, we developed a framework linking transformational leadership to advice provision among organ donation coordinators from medical institutions in China ( N = 316), and examined the underlying mechanisms. Regression modeling showed transformational leadership was significantly and positively associated with coordinators' propensity to provide advice to the families of organ donors, primarily by reducing their perceived risk and enhancing their perceived obligation. A high level of perceived organizational support intensified both the negative relationship between perceived risk and advice giving, and the positive relationship between perceived obligation and advice giving. These results underscore the practical value of optimizing leadership styles, refining incentive structures, and enhancing professional competencies to improve the management efficiency of medical institutions and advance the development of organ donation initiatives.
