Key result
Personalized surgical algorithm for lower extremity lesions linked to a ~2% complication rate.
Why the study?
A personalised approach to treating patients with atherosclerotic lesions of lower extremity arteries considering concomitant pathology and lesion features was needed to improve outcomes.
Cohort (n=386)
May support personalized algorithms in lower extremity revascularization; leaves open randomized confirmation of benefit.
revascularisation; sex and age characteristics of patients; the presence and severity of concomitant diseases such as diabetes mellitus, artery hypertension, coronary heart disease, and chronic heart failure; and damage to other arterial basins. Each of the indicators, depending on the severity, has a numerical equivalent. The values of all indicators are summed.Determination of the tactics of surgical treatment is determined by the total of points: with a total > 5.5, open surgery is chosen; with a total of 5.5 to e 5.5, hybrid interventions are preferred; and with a total of less than e5.5, we choose endovascular interventions. When searching for the criteria included in the algorithm for choosing a method of surgical treatment, attention was paid to those factors that, according to a number of studies, have the greatest weight in predicting the results of various options for revascularisation of peripheral arteries in atherosclerotic lesions. Results: In total, 275 patients were treated in accordance with the declared algorithm.Open surgery was performed in 108 patients (39.3%), endovascular interventions in 96 patients (34.9%), and hybrid technologies in 71 patients (25.8%).The development of complications was recorded in six patients (2.2%): four (1.5%) had infection of the surgical site and two (0.7%) had bleeding in the area of surgical intervention. Not a single patient required the second surgeries or amputations of the treated limb.Of 111 patients who were treated using the prototype method.Open surgery was performed in 12 patients (10.8%), endovascular interventions in 90 (81.1%), and hybrid technologies in eight patients (7.2%).Thrombosis of the surgery segment was recorded in four patients (3.6%), infection of the surgical site in six (5.4%), bleeding in seven (6.3%), repeated reconstructions in 12 (10.8%),and amputations in three (2.7%).Conclusion: A personalised approach to the treatment of patients with atherosclerotic lesions of the arteries of the lower extremities, accounting for the presence and severity of concomitant pathology, along with the clinical picture and features of atherosclerotic lesions of the arteries of the lower extremities, has the best immediate and long term results.

Nana et al. (2022) conducted a cohort in Atherosclerotic lesions of the arteries of the lower extremities (n=386). Personalized algorithm for choosing surgical treatment vs. Prototype method was evaluated on Complications. A personalized algorithm for selecting surgical treatment for lower extremity atherosclerotic lesions resulted in a 2.2% complication rate, with no amputations or repeated surgeries required.
