Multiple risk factors or PVD linked to ~6-fold higher silent ischemia in asymptomatic diabetes.
Why the study?
Silent myocardial ischemia is highly prevalent in diabetic patients and is a predictor of mortality, but its screening applicability using exercise stress test in type 2 diabetic patients with atherogenic risk factors needed assessment.

May inform selective ischemia screening in high-risk diabetes; leaves open need for prospective outcome validation.
Al-Ethary et al. (2020) conducted a cross-sectional in Type 2 Diabetes Mellitus without symptoms of ischemia (n=63). ≥2 atherogenic risk factors or peripheral vascular disease vs. ≤1 atherogenic risk factor was evaluated on Positive exercise stress test (silent myocardial ischemia). In asymptomatic patients with type 2 diabetes, the presence of ≥2 atherogenic risk factors or peripheral vascular disease was associated with a higher rate of silent myocardial ischemia on exercise stress testing (28.6% vs 4.5%).
