Nebulized beta-agonists for acute asthma trigger Takotsubo cardiomyopathy with full recovery at 3 months.
Why the study?
The association between acute asthma exacerbation and takotsubo cardiomyopathy (TTC) is not well recognized, complicating diagnosis and management in elderly patients.

May alert clinicians to Takotsubo risk during catecholamine nebulization for asthma; leaves open causality and incidence.
A 73-year-old patient who was admitted secondary to acute asthma exacerbation that required frequent salbutamol and adrenaline nebulization. Takotsubo cardiomyopathy (TTC) was diagnosed after the new onset of chest pain with modest troponin elevation and normal coronary angiogram. Low ejection fraction and apical akinesia were completely resolved after her symptoms got improved.

Khodari et al. (2023) conducted a case report in Takotsubo cardiomyopathy induced by acute asthma exacerbation (n=1). Acute asthma exacerbation and sympathomimetic therapy (albuterol and adrenaline) was evaluated on Development and resolution of Takotsubo cardiomyopathy (ejection fraction and wall motion). Acute asthma exacerbation treated with albuterol and adrenaline nebulization triggered Takotsubo cardiomyopathy, which completely resolved after 3 months of appropriate management.
