Dobutamine stress echocardiography induces transient ST-segment elevation and hypotension despite non-obstructive coronary arteries.
Why the study?
ST-segment elevation during dobutamine stress echocardiography is commonly linked to significant coronary stenosis, but cases without critical luminal lesions are rare.

Transient ST elevation during DSE without critical CAD warrants vigilance; leaves open mechanistic and management questions for future study.
there have been extremely rare cases in the literature of transmural myocardial infarction with development of ST-segment elevation, occurring in about 0.1% of reported cases.3,9-11 ST-segment elevation during DSE has been commonly associated with significant luminal stenosis and subsequent coronary artery disease.9,12-15 ST-segment elevation without evidence of critical luminal lesions is a significantly uncommon event.13 We report the case of a patient who developed transient ST-segment elevations during DSE.

A 2017 study conducted a case report in Poor functional capacity requiring pre-operative evaluation (n=1). Dobutamine stress echocardiography was evaluated on ST-segment elevation on continuous ECG monitoring. Dobutamine stress echocardiography induced transient 1.5-2.5mm ST-segment elevations and systemic hypotension in a patient with non-obstructive coronary arteries.
