Key result
Severe CKD linked to ~92% slower atherosclerotic plaque growth compared to moderate CKD.
Why the study?
Cardiovascular events are 10 to 100 times more frequent in CKD, but the rate of atherosclerotic plaque growth in severe versus moderate CKD was unclear.
Cohort (n=318)
Absolute Event Rate: 0.4% vs 5%
Paradoxically slower plaque progression in severe CKD; leaves open whether traditional risks outweigh uremia-specific drivers in moderate-to-advanced disease.

Rigatto et al. (2009) conducted a cohort in Chronic Kidney Disease (n=318). Severe CKD (lowest quintile of creatinine clearance < 23 ml/min/1.73 m2) vs. Moderate CKD (highest quintile of creatinine clearance > 43 ml/min/1.73 m2) was evaluated on Rate of atherosclerotic plaque growth. Severe CKD (lowest quintile of CCr) was associated with a lower median rate of atherosclerotic plaque growth (0.4 mm2/yr) compared to moderate CKD (highest quintile; 5.0 mm2/yr).
