A bstract Bartter syndrome (BS) is a rare inherited renal tubular disorder characterized by hypokalemia , metabolic alkalosis, and hyperreninemic hyperaldosteronism, usually presenting in antenatal or childhood periods. Adult-onset BS is uncommon, and unusual clinical manifestations may pose diagnostic challenges. This case series describes three adult patients with BS, who exhibited atypical clinical features. Each patient underwent biochemical and radiological evaluation, and genetic testing was performed when feasible to confirm the diagnosis. Early recognition and timely management are essential to prevent complications and preserve long-term renal function.
