Mucinous adenocarcinoma (MC) of the prostate is a rare variant of prostate cancer, for which no standard treatment has been established. In this case report, we present a 77-year-old man with MC of the prostate who was successfully treated with intensity-modulated radiation therapy (IMRT; total dose of 78 Gy in 39 fractions) and androgen-deprivation therapy for 27 months. Although he maintained a disease-free status, he developed bladder cancer, necessitating radical cystectomy at 62 months after IMRT. The prostate and cystic lesions were also resected and no residual tumor cells of MC of the prostate were identified, which confirmed a pathological complete response. No recurrence was observed at 79 months after IMRT. This case highlights the effectiveness of IMRT for MC of the prostate, providing informative clinical data which may aid clinicians when selecting IMRT as definitive treatment for the management of such patients.
