Increased pericoronary adipose tissue attenuation is linked to early AF recurrence after catheter ablation.

Can artificial intelligence-based pericoronary adipose tissue (PCAT) analysis predict atrial fibrillation recurrence after catheter ablation?
PCAT analysis on coronary CT angiography may serve as a useful imaging biomarker for risk stratification of atrial fibrillation recurrence following catheter ablation.
May flag recurrence risk despite low conventional factors; extends PCAT thresholds but leaves open prospective validation before clinical use.
Atrial fibrillation (AF) recurrence after catheter ablation remains a clinical challenge. We report a 41-year-old man with recurrent AF within six months of radiofrequency ablation despite minimal conventional risk factors, including normal left atrial size and no significant structural heart disease.Pre-ablation coronary computed tomography angiography showed minimal stenosis, but artificial intelligence-based pericoronary adipose tissue (PCAT) analysis revealed increased attenuation around the left circumflex and right coronary arteries, with values above reported recurrence-associated thresholds. This case suggests that PCAT may serve as an additional imaging biomarker for AF recurrence risk stratification.

Zuhri et al. (2026) conducted a case report in Atrial fibrillation recurrence after catheter ablation (n=1). Pericoronary adipose tissue (PCAT) analysis was evaluated on Atrial fibrillation recurrence. Increased pericoronary adipose tissue attenuation was identified in a patient with early recurrent atrial fibrillation after ablation despite lacking conventional risk factors.
