Eplontersen therapy linked to ~24% reduction in volumetric heart-to-lung ratio on SPECT/CT.
Why the study?
The long-term impact of eplontersen on Tc-PYP SPECT/CT uptake in patients with Ala97Ser hereditary transthyretin amyloid cardiomyopathy was not established.
Does eplontersen therapy reduce myocardial tracer uptake on Tc-PYP SPECT/CT in patients with Ala97Ser hereditary transthyretin amyloid cardiomyopathy?

Does eplontersen therapy reduce myocardial tracer uptake on Tc-PYP SPECT/CT in patients with Ala97Ser hereditary transthyretin amyloid cardiomyopathy?
Eplontersen therapy is associated with a significant long-term reduction in myocardial tracer uptake on Tc-PYP SPECT/CT in patients with hATTR-CM.
Serial Tc-PYP SPECT/CT may track eplontersen response in hATTR-CM; leaves open utility pending larger prospective studies.
Background: Tc-PYP) single-photon emission computed tomography/computed tomography (SPECT/CT) uptake in patients with hATTR-CM. Methods: Tc-PYP SPECT/CT parameters were assessed. Results: Five patients met the inclusion criteria. The mean follow-up durations were 22.4 months (first follow-up) and 37.5 months (second follow-up). A significant reduction in the volumetric heart-to-lung ratio was observed from baseline to first follow-up (3.75 ± 0.53 to 2.94 ± 0.38, p = 0.010) and to second follow-up (3.75 ± 0.53 to 2.86 ± 0.39, p = 0.009). A further reduction between the first and second follow-up was also noted (2.94 ± 0.38 to 2.86 ± 0.39, p = 0.025). Conclusions: Tc-PYP SPECT/CT imaging demonstrated a significant reduction in myocardial tracer uptake over a mean follow-up of 37.5 months in patients with hATTR-CM undergoing eplontersen therapy.

Hung et al. (2026) conducted an observational in Ala97Ser Hereditary Transthyretin Amyloid Cardiomyopathy (n=5). Eplontersen vs. Baseline was evaluated on Volumetric heart-to-lung ratio (p=0.009). Eplontersen therapy significantly reduced the volumetric heart-to-lung ratio on Tc-PYP SPECT/CT from 3.75 at baseline to 2.86 over 37.5 months (p=0.009).
