Active exercise yields ~32 percentage points fewer complex PVC morphologies compared with early recovery.
Why the study?
Despite the connection between physical exercise and ARVC, there are no data on the safety and suitability of exercise for patients with plakophilin-2 variants.
Does the type and phase of physical exercise affect the burden of premature ventricular contractions and cardiometabolic load in patients with ARVC and plakophilin-2 variants?

Does the type and phase of physical exercise affect the burden of premature ventricular contractions and cardiometabolic load in patients with ARVC and plakophilin-2 variants?
In patients with ARVC and plakophilin-2 variants, the arrhythmic burden peaks during early recovery rather than active exercise, and small muscle mass resistance exercises appear to be safer with no significant increase in PVCs compared to rest.
Recovery after exercise may carry higher arrhythmic risk than activity itself in ARVC; establishes new direction for tailored exercise recommendations.
BACKGROUND: Despite the connection between physical exercise and arrhythmogenic right ventricular cardiomyopathy (ARVC), there are no data on the safety and suitability of exercise for patients. OBJECTIVE: To quantify the incidence and complexity of premature ventricular contractions (PVCs) and cardiometabolic load during different exercises in patients with ARVC with pathogenic plakophilin-2 variants. METHODS: 25 participants with definite ARVC (9 female and 16 male) performed different exercises while cardiorespiratory function was monitored. Resistance exercises included 2-legged squats and unilateral biceps curls, whereas endurance exercises included walking and cycling at a heart rate of 80, 100, and 120 bpm. Each activity was followed by 10 minutes of recovery. RESULTS: PVC burden more than doubled (P < .001) during the early recovery phase than in the active exercise phases, as did the occurrence of ≥ 3 PVC morphologies (52% vs 84%, P = .03). Only unilateral biceps curls did not increase PVC burden compared with rest (P = .361). Resistance exercises combined lower cardiac load and arrhythmic burden for comparable effort levels. Exercise intensity classification showed large fluctuations, from "light" to "high intensity" for the same task, depending on the metric used. CONCLUSIONS: Early recovery monitoring is essential to properly assess the arrhythmic burden of exercise in plakophilin-2 ARVC. Small muscle mass exercises show the lowest PVC burden and cardiac load-surrogate markers of arrhythmic risk and disease progression-and therefore offer a promising path for prescribing exercise in this population. Exercise intensity guidelines for ARVC are insufficiently clear and should be refined.

Beltrami et al. (2026) conducted a cross-sectional in Arrhythmogenic right ventricular cardiomyopathy (ARVC) with pathogenic plakophilin-2 variants (n=25). Physical exercise (active exercise phases) vs. Early recovery phase was evaluated on Occurrence of ≥ 3 PVC morphologies (p=0.03). Active physical exercise resulted in a lower occurrence of ≥3 PVC morphologies compared to the early recovery phase (52% vs 84%, P=0.03), and PVC burden more than doubled during recovery.
