Self-expanding TAVR offers excellent hemodynamics but retains ongoing risks of pacemakers and paravalvular leak.
Why the study?
Self-expanding transcatheter valves have evolved with randomized evidence across surgical risk, but historical liabilities and technical challenges remain to be addressed.
Self-expanding TAVR platforms offer excellent hemodynamics and flexibility, though challenges such as pacemaker requirements and paravalvular leak persist.
Self-expanding transcatheter valves have evolved significantly into mature therapeutic platforms supported by randomized evidence across the spectrum of surgical risk. The most consistent advantages of self-expanding transcatheter aortic valve replacement remain excellent hemodynamics, especially in small annuli, and the technical flexibility conferred by controlled deployment and recapturability. The principal historical liabilities-pacemaker implantation, paravalvular leak, and coronary reaccess-have improved substantially with new devices and refined implantation technique, although none has been eliminated.

Lamelas et al. (2026) conducted a review in Aortic valve disease. Self-expanding transcatheter aortic valve replacement was evaluated. Self-expanding transcatheter aortic valve replacement offers excellent hemodynamics and technical flexibility, though historical liabilities like pacemaker implantation and paravalvular leak remain.
