Left atrial appendage ablation successfully eliminates refractory epicardial PVCs using CT-guided noninvasive 3D mapping.
Why the study?
Ablation of ectopy originating from the left ventricle can be challenging when the origin is in the epicardial LV summit area and conventional approaches fail.

LAA ablation may succeed for refractory epicardial PVCs after failed conventional attempts; leaves open generalizability and safety in larger cohorts.
Ablation is an established treatment for ectopy originating from the left ventricle (LV). We report on a case of noninvasive 3-dimensional mapping locating the origin precisely in the epicardial LV summit area. However, after failed attempts from LV and epicardially, ablation via the left atrial appendage was finally successful. (Level of Difficulty: Advanced.)

Obergassel et al. (2020) conducted a case report in Premature ventricular contractions and nonsustained ventricular tachycardia (n=1). Catheter ablation via the left atrial appendage guided by noninvasive 3D mapping was evaluated on Elimination of premature ventricular contractions. Catheter ablation via the left atrial appendage, guided by noninvasive 3D mapping and a CT roadmap, successfully eliminated refractory epicardial premature ventricular contractions.
