Blockchain systems face critical security vulnerabilities across all architectural layers, with over $1.8 billion stolen in 2023 alone. This systematic review analyzes 140 peer-reviewed studies (2018–2025) to provide a comprehensive taxonomy of blockchain vulnerabilities, detection methods, and mitigation strategies. We categorize 30+ distinct attack types across five architectural layers (Hardware, Data, Network, Consensus, Application) and three critical applications (Smart Contracts, Decentralized Exchanges, Supply Chains). Our quantitative analysis reveals severe research imbalance: the Application layer receives 58% of research attention with 45+ detection tools, while foundational Hardware (9%) and Data layers (6.7%) remain critically underexplored. We systematically compare 25+ vulnerability detection tools, identifying their strengths, limitations, and deployment contexts. Unlike existing surveys focusing on isolated aspects, we provide integrated detection mitigation mappings and analyze vulnerability propagation across layers. We identify nine security paradigms including AI-powered agents, zero-knowledge proofs, and post-quantum cryptography with assessment of maturity levels and adoption timelines. This work provides researchers and practitioners with an actionable roadmap for understanding current vulnerabilities, selecting appropriate tools, and anticipating future challenges in blockchain ecosystems.
