Does exercise intervention improve vascular elasticity in community-dwelling older adults aged 65 years and older?
Meta-Analysis
Does exercise intervention improve vascular elasticity in community-dwelling older adults aged 65 years and older?
Exercise interventions may significantly improve upper- and lower-limb pulse wave velocity in older adults, though findings are limited by heterogeneity and potential publication bias.
The purpose of this study was to systematically review and quantitatively evaluate the effects of exercise interventions on vascular elasticity in community-dwelling older adults in Korea through a meta-analysis. A systematic literature search was conducted in accordance with the PRISMA 2020 guidelines using the RISS, KCI, DBpia, e-Article, and KISS databases. Domestic studies involving exercise interventions for adults aged 65 years and older were included. A total of eight studies met the inclusion criteria. Effect sizes were calculated using the standardized mean difference (Hedges' g), and a random-effects model was applied. Heterogeneity among studies was assessed using Cochran's Q test and Higgins' I² statistic, and publication bias was examined using funnel plots, the trim-and-fill, Egger’s test and sensitivity analysis method. The results showed that exercise interventions produced a significant pooled effect on upper- and lower-limb pulse wave velocity (SMD=2.16, 95% CI=0.90–3.43), although heterogeneity among the included studies was substantial. In contrast, no significant effect was observed for brachial-ankle pulse wave velocity (baPWV) (SMD=-0.08, 95% CI=-0.60 to 0.44). However, Egger’s test and the trim-and-fill analysis indicated potential publication bias; therefore, the findings should be interpreted with caution. These findings suggest that exercise interventions may have beneficial effects on upper- and lower-limb PWV in Korean older adults. This study systematically synthesized the available domestic evidence on exercise interventions and vascular elasticity and provides quantitative evidence that may serve as a useful reference for evidence-based exercise prescription and future intervention studies aimed at promoting vascular health in older adults.

Hwang et al. (2026) conducted a meta-analysis in Vascular elasticity. Exercise interventions was evaluated on Upper- and lower-limb pulse wave velocity (SMD 2.16, 95% CI 0.90-3.43). Exercise interventions produced a significant improvement in upper- and lower-limb pulse wave velocity in older Korean adults (SMD 2.16; 95% CI 0.90-3.43).
