Introduction: Pleomorphic adenomas (PA) are benign tumors of the salivary glands that have the potential to develop considerably if therapy is not performed. Case Presentation: Present study report a case of 75 years old male patient having swelling in upper sublabial region. Clinical investigation of the patient showed a non-mobile, firm, non-tender swelling on the mucosal side of the lip. Patient was presenting the symptoms for 7 months and histopathology report confirm after 2 weeks. Ultrasonography showed a well-defined heterogeneous hypoechoic lesion of size measuring 4×3 cm with multiple internal cystic areas and vascularity, suggestive of a neoplastic lesion. Cellular smears of epithelial cells with mild pleomorphism admixed with chondromyxoid stromal fragments in mucoid background were revealed through fine needle aspiration cytology (FNAC).Conclusion: Pleomorphic adenoma (PA) of lip is a form of rare tumor, so diagnosing it demands a lengthy follow-up and a high index of suspicion. Fine needle aspiration cytology (FNAC) is a good preoperative diagnostic tool and for prevention of the recurrence, complete surgical excision is the ideal course of remedy.
