Q fever and rickettsial diseases are underrecognized causes of fever of unknown origin in China, with Coxiella burnetii and Orientia tsutsugamushi predominating. We identify Rickettsia bellii-like sequences in human clinical specimens and highlight unexpected pediatric detections that warrant further investigation of possible exposure patterns, supporting enhanced surveillance and One Health strategies.
