Fertility preference significantly influences population dynamics in the northeastern states of India. This study examines fertility preferences among currently married women aged 15–49 using four rounds of National Family Health Survey (1998–2021). Based on binary logistic regression analysis, there is an increasing preference for stopping childbearing after one or two children, reflecting the adoption of small family norms. The total fertility rate also declined across most states. Education, wealth, religion, gender composition and parity significantly shape fertility preferences in the region. Persistent son preference and socio-economic progress continue to influence ongoing fertility transitions in the northeastern states of India.
