Vocal fold paralysis refers to complete immobility caused by neurological injury, and it can result in considerable health challenges such as voice disorders, difficulty swallowing, and aspiration. Otolaryngologists may encounter challenges in thoroughly evaluating the underlying cause of this condition due to the complexity of remembering all potential causes in one go. To simplify understanding the causes, a systematic approach categorizes the etiology of vocal fold palsy into areas such as the neck, chest, esophagus, central nervous system, or systemic diseases. This article presents a discussion of the etiologies behind vocal fold paralysis and approach to a patient with vocal fold paralysis in a system-based approach. By using this categorization, there is a higher likelihood of identifying the causative factor behind vocal fold paralysis, enhancing the relevance of the diagnosis and treatment.
