Peripheral artery disease (PAD) is an increasing global health burden. Endovascular treatment is central to revascularization, yet outcomes remain suboptimal in complex lesions and advanced chronic limb-threatening ischemia. Several recently marketed or investigational devices have been developed to improve lesion crossing, vessel preparation, drug delivery, patency, and limb outcomes. This state-of-the-art review summarizes the design rationale and clinical evidence for these emerging endovascular technologies. Because much of the available evidence comes from small, single-arm studies with heterogeneous endpoints, technical success and short-term patency should not be equated with established long-term clinical benefit.
