Off-hours mechanical thrombectomy yields similar 90-day functional outcomes to on-hours treatment despite treatment delays.
Why the study?
Mechanical thrombectomy is the standard of care for acute large vessel occlusion, but off-hours treatment may be delayed due to limited staffing, prompting a comparison of on-hours and off-hours outcomes.
Does performing mechanical thrombectomy during off-hours compared to on-hours affect 90-day functional outcomes (mRS 0–3) and procedural time metrics in patients with acute anterior circulation large vessel occlusion?

Does performing mechanical thrombectomy during off-hours compared to on-hours affect 90-day functional outcomes (mRS 0–3) and procedural time metrics in patients with acute anterior circulation large vessel occlusion?
Although off-hours presentation for mechanical thrombectomy resulted in significant in-hospital delays from imaging to arterial puncture due to on-call staffing workflows, 90-day functional outcomes (mRS 0–3) and recanalization rates were comparable to on-hours treatment.
May support off-hours MT feasibility despite delays; leaves open whether workflow optimizations improve outcomes in prospective studies.
Objective: Mechanical thrombectomy (MT) is the standard of care for acute large vessel occlusion (LVO); however, off-hours treatment may be delayed because of limited staffing. We compared MT outcomes between on-hours and off-hours at a single center and investigated factors associated with 90-day modified Rankin Scale (mRS) 0–3.

Kanazawa et al. (2026) conducted a cohort in Acute anterior circulation large vessel occlusion stroke (n=101). Off-hours presentation for mechanical thrombectomy vs. On-hours presentation (9:00-17:00 Mondays-Saturdays, excluding holidays) was evaluated on 90-day modified Rankin Scale (mRS) 0-3 (OR 0.917, 95% CI 0.386-2.179, p=0.845). Off-hours mechanical thrombectomy had similar 90-day mRS 0-3 rates to on-hours treatment (45.5% vs 50.0%; adjusted OR 0.917, p=0.845) despite longer door-to-puncture times.
