Key result
Prior COVID-19 in STEMI is linked to ~2.4-fold higher mCRP levels at 6 months.
Why the study?
The study aimed to analyze in-hospital and post-hospital serum hsCRP and mCRP up to 6 months in patients with acute STEMI and previous COVID-19.
Does previous COVID-19 infection 1.5 to 6 months prior to ACS increase serum hsCRP and mCRP levels up to 6 months after STEMI compared to no previous COVID-19?
Cohort (n=140)
Does previous COVID-19 infection 1.5 to 6 months prior to ACS increase serum hsCRP and mCRP levels up to 6 months after STEMI compared to no previous COVID-19?
Patients with STEMI and a recent history of COVID-19 exhibit persistently elevated mCRP for up to 6 months and elevated hsCRP in the first month, indicating a more severe and prolonged post-infarction inflammatory reaction.

Prior COVID-19 may prolong post-STEMI inflammation; hypothesis-generating and should not yet change practice.
Objective. To analyze in-hospital and post-hospital (up to 6 months) serum hsCRP and mCRP in patients with acute ST-segment elevation myocardial infarction (STEMI) and previous COVID-19. Material and methods. There were 140 patients with STEMI stratified into 2 groups: 1 — 52 STEMI patients with COVID-19 1.5—6 months before acute coronary syndrome (ACS), 2 — 88 patients with STEMI and no previous COVID-19. PCI with stenting of infarct-related artery was performed on the first day. Serum hsCRP and mCRP were determined in 2 and 10 days, 1 and 6 months after MI. Results. Serum mCRP was higher in the main group at all time points: by 5.09 times after 2 day, 13.50 times after 10 days, 5.70 times after 1 month and 2.41 times after 6 months. In-hospital serum hsCRP was higher in the main group (by 2.05 times after 2 days and 3.36 times after 10 days). Serum hsCRP after 1 and 6 months was equivalent in both groups. Dynamics of serum mCRP and hsCRP was multidirectional after 10 days (mCRP increased, hsCRP did not change in the main group, and both parameters decreased in the control group). After 1 and 6 months, dynamics of these parameters was unidirectional and positive in both groups. Conclusion. Patients with STEMI and previous COVID-19 are characterized by higher concentration of mCRP in the first 6 months and hsCRP in the first month of treatment. This indicates more severe inflammatory reaction in the first 6 months after STEMI.

Mikhin et al. (2026) conducted a cohort in Acute ST-segment elevation myocardial infarction (STEMI) (n=140). Previous COVID-19 (1.5-6 months before ACS) vs. No previous COVID-19 was evaluated on In-hospital and post-hospital (up to 6 months) serum hsCRP and mCRP levels. Previous COVID-19 in STEMI patients was associated with higher serum mCRP at all time points up to 6 months (13.50-fold at day 10; 2.41-fold at 6 months) and higher in-hospital hsCRP.
