Why the study?
The Myval balloon-expandable valve was recently marketed in Peru and is primarily used for aortic valve replacement, with these cases representing the first experience of its use for pulmonary valve implantation in the country.
Is transcatheter pulmonary valve replacement with the Myval balloon-expandable valve feasible and hemodynamically effective in adolescent patients with right ventricular outflow tract dysfunction after tetralogy of Fallot or Fallot-type double outlet right ventricle repair?

Is transcatheter pulmonary valve replacement with the Myval balloon-expandable valve feasible and hemodynamically effective in adolescent patients with right ventricular outflow tract dysfunction after tetralogy of Fallot or Fallot-type double outlet right ventricle repair?
This case series reports the first three successful off-label transcatheter pulmonary valve replacements using the Myval balloon-expandable valve in Peru, demonstrating technical feasibility and favorable acute hemodynamic outcomes in adolescents with postoperative RVOT dysfunction.
Demonstrates short-term feasibility of Myval TPVR post-TOF repair; leaves open durability, safety, and generalizability in larger cohorts.
Percutaneous pulmonary valve implantation is an alternative to surgical replacement for patients with right ventricular outflow tract dysfunction—such as those who have undergone surgery for tetralogy of Fallot, truncus arteriosus, or pulmonary atresia with ventricular septal defect and a right ventricle-to-pulmonary artery conduit, as well as patients with severe pulmonary regurgitation following pulmonary valvuloplasty. In our country, the Melody® balloon-expandable valve (Medtronic), the Pulsta™ self-expanding valve, and, more recently, the Myval™ balloon-expandable valve are currently marketed. The Myval™ is primarily used for aortic valve replacement but is also used for pulmonary valve replacement or valve-in-valve replacement of the mitral or tricuspid valve. We present the first three cases of Myval™ pulmonary valve implantation performed in Peru in adolescent patients with right ventricular outflow tract dysfunction following surgery for tetralogy of Fallot.

Cabrera et al. (2026) conducted a case report in Right ventricular outflow tract dysfunction following surgical repair of tetralogy of Fallot or Fallot-type double outlet right ventricle (n=3). Transcatheter pulmonary valve replacement with Myval balloon-expandable valve was evaluated on Procedural feasibility and early hemodynamic success (post-implant mean transvalvular gradient by echocardiography).
