The increasing accessibility of online pornography has led to earlier exposure among children and adolescents, raising concerns about its potential long-term effects on sexual development, behaviour and mental health. This systematic literature review synthesises current evidence on the relationship between early exposure to pornography and later pornography consumption. A comprehensive search of six electronic databases (MEDLINE, PsycINFO, Sociological Abstracts, Embase, ASSIA, Web of Science) identified 17 studies that met the inclusion criteria, which required individuals to have experienced early exposure to pornography and reported subsequent pornography consumption. Eligible quantitative studies (longitudinal, cohort, case-control, cross-sectional), published in English from 1993 onwards, were identified and screened against inclusion and exclusion criteria. Across studies, earlier age of first exposure was consistently associated with higher frequency of use, greater risk of compulsive or problematic consumption and progression towards more extreme or deviant content in adolescence and adulthood. Developmental and gender differences emerged, with males generally exposed earlier and exhibiting stronger associations between early exposure and problematic use, although similar attitudinal effects were noted among females. Mediating mechanisms included internalisation of sexual scripts, perceived realism, shame and emotional distress, while moderating factors encompassed impulsivity, sensation-seeking, family stability, religiosity and cultural context. Overall, findings indicate that early exposure to pornography may shape enduring behavioural and cognitive trajectories, with outcomes influenced by individual, developmental and social factors. Findings and future directions are discussed in relation to implications for practice, policy and future research.
