The management of donor data and donation reporting at Masjid Raya Jorong Pasar Tiku, Nagari Tiku Selatan, Kecamatan Tanjung Mutiara, Kabupaten Agam, West Sumatra, was previously carried out manually, causing difficulties in data retrieval, recording accuracy, and report preparation. This study aims to design and develop a web-based information system for donor data collection and donation reporting. The research used the Waterfall development model with observation, interviews, and documentation as data collection techniques. System requirements were analyzed and modeled using UML, while the application was implemented using PHP Native and MySQL. Functional verification was conducted using black-box testing. The resulting system provides an administrator login, donor management, donation data entry and verification, mosque expenditure management, gallery management, user management, and donation/financial recapitulation reports. Testing covered eight main functions, namely the main page, login, administrator management, donation registration, donor management, mosque expenditure, gallery input, and recapitulation. All tested functions produced the expected outputs and were recorded as successful. The system therefore provides a structured digital mechanism for managing donor data and producing donation reports at the research site.
