Rheumatoid arthritis (RA) and osteoarthritis (OA) play an important role in the spectrum of modern human diseases. Currently, the pathogenesis of these disorders is considered regarding epigenetic regulation. Differences in the regulation of gene expression result in an altered immune response, leading to chronic joint inflammation. This review analyzes several epigenetic mechanisms associated with changes in proand anti-inflammatory factors that contribute to chronic immune-mediated inflammation. We conducted a literature search in the eLibrary, PubMed, CyberLeninka, and Frontiers medical databases using the search terms “epigenetics”, “epigenetic regulation”, “epigenetics of rheumatoid arthritis”, and “epigenetics of osteoarthritis”. Current data on the influence of epigenetic regulation on the development and prognosis of RA and OA are presented. The literature search covered the period from May 1994 to January 2025. Initially, 210 references were identified. Articles without freely available full text and studies unrelated to the objective of the review were excluded from the analysis. A total of 41 articles meeting the objective of this review were analyzed.
