The cerebrovascular diseases represent a critical medical social problem in the Russian Federation, conditioning significant demographic losses and economic damage due to high mortality and disability of able-bodied population. This article presents comprehensive analysis of epidemiological indicators of acute impairments of cerebral circulation in Russia in 2018–2026 as compared with international data. It is established that despite decreasing of acute lethality due to expansion of vascular centers and implementation of thrombolytic therapy, the overall social economic burden of stroke remains critically high due to increase of the number of patients with residual neurological deficits and persistent disability. The critical analysis of existing organizational models of medical rehabilitation revealed systemic problems of accessibility and continuity of care, deficiency of qualified specialists and insufficient funding of programs of long-term support. The study substantiated necessity of priority-driven development of three-stage rehabilitation system with multidisciplinary approach, expansion of infrastructure of rehabilitation institutions in peripheral regions and strengthening of population programs of primary prevention in able-bodied individuals to achieve significant decreasing of social economic losses.
