Slow morbidity reporting and reliance on static tables in the medical records unit can hinder disease trend analysis and managerial decision-making. This study aims to design a web-based outpatient morbidity reporting information system dashboard based on the International Classification of Diseases Tenth Revision (ICD-10) at RSUD Pagelaran. The research methodology used a descriptive qualitative approach with the Waterfall system development model, consisting of requirements analysis, system design, implementation, testing, and maintenance. The system was modeled using the Unified Modeling Language (UML). The results of the study are a web-based dashboard design capable of presenting interactive clinical data visualizations, including morbidity trend graphs, disease distribution tables, and indicators of the top 10 most common diseases. The system is designed to address the previous problem of semi-manual monthly visit data recapitulation using Microsoft Excel, which is time-consuming and prone to human error. In conclusion, the proposed dashboard is expected to improve staff work efficiency, reporting accuracy, and support healthcare quality monitoring quickly and accurately
