Forest health is a key indicator for assessing sustainable ecosystem management, particularly in conservation areas under land conversion pressure. This study aimed to analyze forest health conditions on cultivated land of Gapoktan Maju Lestari in Tahura Wan Abdul Rachman using the Forest Health Monitoring method. Data collection was conducted on six cluster plots with 5% sampling intensity of the 44 ha cultivated area. Parameters measured included productivity (Basal Area/LBDs), vitality (tree damage/CLI and crown condition/VCR), biodiversity (Shannon-Wiener Index/H'), and site quality (soil pH). Results showed LBDs values ranged from 50.74–208.69 m²/ha, CLI 4.07–5.43 (good category), VCR 2.80–3.58, H' 0.18–0.22 (low category), and soil pH 6.47–7.13 (slightly acidic to neutral). The average final forest health value (NKHr) was 4.73, classified as Moderate. This indicates the forest ecosystem is functioning reasonably well despite ecological pressures. Increasing tree species diversity through mixed planting systems is needed to improve long-term ecosystem stability.
