Colorectal cancer is one of the most common types of gastrointestinal cancer, with relatively high incidence and mortality rates. Several factors, such as age, gender, and tumor location, are believed to influence the degree of histopathological differentiation associated with patient prognosis. Determine the relationship between age, gender, and tumor location and histopathological differentiation grade in colorectal cancer patients at RSD Gunung Jati Cirebon from 2019 to 2024. This study employed an analytical observational design with a cross-sectional approach. Secondary data were obtained from the medical records of 97 colorectal cancer patients. Spearman's rank test was used to analyze the data and assess the relationship between the variables. Most patients were in the 60+ (47,42%) and 46–59 (38,14%) age groups, and 64.9% were male. The most common tumor location was the rectum (30,9%), followed by the ascending colon (26,8%) and the sigmoid colon (25,8%). The most common histopathological differentiation grade was moderately differentiated (75.26%), while poorly differentiated accounted for 24,74%; no cases of well-differentiated were found. The results of the analysis showed no significant association between age (p = 0,101) or gender (p = 0.439) and histopathological differentiation grade. However, there was a significant association between tumor location and histopathological differentiation grade (p = 0,001). Conclusion: Of the three studied variables, only tumor location showed a significant association with the histopathological differentiation grade of colorectal cancer. Age and gender did not show a significant association. Keywords: Colorectal Cancer, Age, Gender, Tumor Location, Histopathological Differentiation.
