Key result
Renal artery stenosis surgery achieves normotension in ~54% of patients, correlating best with renin ratios.
Why the study?
Do pre- and peroperative physiological and hemodynamic evaluations correlate with blood pressure normalization after >1 year follow-up in hypertensive patients undergoing surgery for renal artery stenosis?

Do pre- and peroperative physiological and hemodynamic evaluations correlate with blood pressure normalization after >1 year follow-up in hypertensive patients undergoing surgery for renal artery stenosis?
Preoperative renal vein renin ratios and peroperative hemodynamic measurements (pressure gradient, blood flow increase, and resistance ratios) correlate well with long-term blood pressure normalization following surgery for renal artery stenosis.
The operative results after a more than 1 year follow-up were compared with pre-and peroperative investigations on 13 patients with angiographically verified renal artery stenosis. Seven patients became normotensive. Determination of plasma renin activity ratio in the renal veins and the ratio of renin release from the kidneys gave the best correlation to the operative results. Separate clearance studies are of importance if the contralateral kidney is damaged. One patient who underwent reconstruction for renal artery stenosis did not become normotensive until the arteriolosclerotic contralateral kidney was excised. Peroperative determination of mean pressure gradient, increase in renal blood flow, and resistances over the stenosis compared to that of the parenchyma also gave good correlation to the results at 1 year follow-up.

Božovi et al. (1971) conducted a cohort in Hypertension with angiographically verified renal artery stenosis (n=13). Pre- and peroperative evaluation and surgical reconstruction for renal artery stenosis was evaluated on Achievement of normotension at >1 year postoperative follow-up. Surgical treatment of renal artery stenosis resulted in normotension in 7 of 13 patients (53.8%) at >1 year, correlating best with renal vein plasma renin activity and renin release ratios.
