Pemphigus vulgaris is a rare autoimmune bullous disorder in adolescents and is frequently misdiagnosed in its early stages. We report the case of a 14-year-old girl who presented with multiple flaccid bullae associated with oral erosions. The diagnosis of adolescent pemphigus vulgaris was confirmed based on histopathological findings and direct immunofluorescence studies. The patient is currently on dexamethasone-azathioprine pulse therapy with a good clinical response. This case highlights the importance of early recognition and appropriate clinicopathological correlation in diagnosing this condition in the adolescent age group.
