Loading and unloading delays on tankers may arise before berthing or during cargo transfer. This study analyzes the causes, impacts, and dominant factors of operational delays involving MT. Klasogun at Fuel Terminal Camplong. A qualitative descriptive method was applied through observation, interviews, and documentation, followed by Root Cause Analysis (RCA) and a Fishbone diagram. Two groups of constraints were identified. An external factor occurred when a tugboat operated with limited propulsion capability, delaying the berthing process by about two hours. An internal factor occurred when Cargo Oil Pump (COP) No. 2 showed leakage and pressure loss due to a damaged mechanical seal, requiring cargo operations to stop for about four hours while the component was replaced and the system checked. RCA linked the failure to high operating hours, insufficient preventive maintenance, and incomplete pre-operation inspection. Equipment-related problems were the dominant cause. Stronger preventive maintenance, pre-operation checks, tugboat readiness, and ship-terminal coordination are therefore required to reduce delays while maintaining operational safety.
