Surgery and targeted antibiotics achieve a favorable outcome in severe teenage aortic valve endocarditis.
Why the study?
Infective endocarditis in children is a rare and severe disease that is particularly difficult to diagnose in the absence of known heart valve changes, especially when caused by staphylococcal infection with an acute course.
Case Report (n=1)
This case highlights that acute Staphylococcus aureus infective endocarditis of a native aortic valve in an adolescent can present as purulent meningitis and cause rapid valvular destruction, requiring combined surgical and targeted antibacterial therapy for a favorable outcome.

Alerts clinicians to purulent meningitis as possible presentation of acute S. aureus aortic endocarditis in adolescents; hypothesis-generating for combined surgical-antibiotic therapy.
Infective endocarditis (IE) in children is a rare but severe, potentially disabling and fatal disease. Diagnosis and treatment of patients with IE requires vigilance and the engagement of doctors of different specialties. The most difficult cases of IE to diagnose in children are in the absence of previously known changes in the heart valves, especially in cases of staphylococcal etiology and an acute course of the disease. This paper presents a clinical case of acute IE of a native aortic valve in a teenager, which clinically debuted as purulent meningitis associated with staphylococcal infection (Staphylococcus aureus). The aggressive infectious and destructive process in the heart resulted in the development of a fibrous ring abscess and subtotal destruction of the aortic valve. The combination of surgical treatment and targeted antibacterial therapy let to favourable outcome.

Korovina et al. (2026) conducted a case report in Acute infective endocarditis of a native aortic valve (Staphylococcus aureus) presenting as purulent meningitis (n=1). Surgical treatment and targeted antibacterial therapy was evaluated on Clinical outcome. Surgical treatment and targeted antibacterial therapy led to a favorable outcome in 1 teenager with acute Staphylococcus aureus native aortic valve endocarditis and ring abscess.
