Bentracimab rapidly reverses ticagrelor to restore haemostasis in life-threatening bleeding and emergent CABG.
Why the study?
Ticagrelor improves ischaemic outcomes in atherosclerotic cardiovascular disease at the cost of increased bleeding risk, and patients on potent antiplatelet regimens may require rapid restoration of haemostatic activity during life-threatening haemorrhage or emergent surgery.
Does bentracimab effectively reverse ticagrelor P2Y12 inhibition and restore haemostasis in patients presenting with life-threatening haemorrhage or requiring emergent surgery?

This case-based review highlights the clinical utility of bentracimab for rapid reversal of ticagrelor antiplatelet activity in patients experiencing life-threatening haemorrhage or requiring emergent surgery.
INTRODUCTION: inhibitor that improves ischaemic outcomes in atherosclerotic cardiovascular disease, including the prevention of stent thrombosis after percutaneous coronary intervention, at the cost of increased bleeding risk. AREAS COVERED: inhibitors, treatment options for reversing their effects, and describes a novel ticagrelor reversal agent in two cases of life-threatening bleeding, one related to gastrointestinal haemorrhage and one associated with emergent coronary artery bypass graft surgery. EXPERT OPINION: inhibitors. Patients treated with potent antiplatelet regimens may require rapid and effective restoration of haemostatic activity in addition to supportive measures in those presenting with life-threatening haemorrhage or requiring emergent surgery. We summarise the current available treatment options to reverse antiplatelet activity and highlight the role of ticagrelor reversal with bentracimab in improving haemostasis in emergency settings.

Dirir et al. (2026) conducted a review in Life-threatening haemorrhage or emergent coronary artery bypass graft surgery in patients treated with ticagrelor (n=2). Bentracimab (ticagrelor reversal agent) was evaluated on Restoration of haemostatic activity / reversal of antiplatelet activity. Bentracimab for ticagrelor reversal is highlighted in 2 cases of life-threatening bleeding (gastrointestinal haemorrhage and emergent CABG) to rapidly restore haemostasis.
