Modern perfusion leadership drives clinical governance, quality improvement, and workforce development to advance patient safety.
Why the study?
The expanding scope and complexity of contemporary clinical perfusion services have made effective leadership increasingly important for creating the systems, culture, and strategic direction needed to support safe, evidence-based patient care.
Highlights the expanding role of perfusion leaders in driving quality improvement, standardization, innovation, and clinical governance across contemporary extracorporeal and cardiac surgical services.

Expanded perfusion roles now span ECMO and transplantation; leaves open optimal training and staffing models for complex departments.
Clinical perfusion has evolved considerably over the past several decades. While the fundamental responsibility of the perfusionist remains the safe delivery of cardiopulmonary bypass and extracorporeal support, the environment in which perfusion services operate has become increasingly complex.Contemporary perfusion departments support not only cardiac surgery but also extracorporeal membrane oxygenation (ECMO), mechanical circulatory support, organ transplantation, structural heart interventions, blood conservation programs, quality improvement initiatives, research activities, education, and the implementation of emerging technologies. As the scope and complexity of these services continue to expand, effective leadership has become increasingly important. At the center of this evolution is the perfusion leader.Whether serving as a chief perfusionist, manager, supervisor, lead, program director, or senior clinician, perfusion leaders are increasingly responsible for creating the systems, culture, and strategic direction that support safe, effective, and evidence-based patient care.Their influence extends far beyond the operating room, impacting quality, innovation, workforce development, clinical governance, and ultimately patient outcomes. The key domains of modern perfusion leadership are summarized in Figure 1.
