Robotic-assisted perimembranous VSD closure appears feasible in select young adults; leaves open need for comparative trials before wider adoption.
Background Robotic-assisted cardiac surgery represents an alternative approach for ventricular septal defect (VSD) closure, with reported advantages in visualization and precision. Case Presentation An 18-year-old male presented with dyspnea and a grade 4 harsh pansystolic murmur. Transthoracic echocardiography identified a perimembranous VSD with a muscular rim (17 mm), left to right shunt, with a peak gradient of 81 mmHg and associated right coronary cusp prolapse without aortic regurgitation. Transesophageal echocardiography (TEE) confirmed the findings. Surgical Technique The patient underwent robotic-assisted VSD closure via a peripheral cardiopulmonary bypass approach using the Da Vinci X (Intuitive Surgical Inc.). Six strategically placed robotic and assistant ports were positioned using right-sided intercostal access. After pericardiotomy, right atriotomy, and cardioplegic arrest of the heart, the VSD was visualized and closed with a glutaraldehyde-treated pericardial patch 5-0 polypropylene sutures as shown in Figure 1. Post-operative TEE confirmed complete VSD closure with intact tricuspid valve function. Outcome The patient's postoperative course was uneventful with no residual defect and no complications. This case demonstrates the feasibility of robotic-assisted VSD closure in appropriately selected candidates, highlighting the technical advantages of robotic assistance in a case of perimembranous VSD.
