Transcatheter Occlutech PLD closure resolves aortic paravalvular leak symptoms and cuts LDH by ~51%.
Why the study?
Most regional paravalvular leak closures rely on non-dedicated devices not purposefully engineered for this pathology, motivating evaluation of the dedicated Occlutech PLD Occluder device.
Does transcatheter closure using the dedicated Occlutech PLD Occluder improve procedural outcomes, symptoms, and haemolytic anaemia in a patient with a complex aortic paravalvular leak?

This case report demonstrates the feasibility and successful clinical outcome of using a dedicated Occlutech PLD occluder for transcatheter closure of a complex aortic paravalvular leak, resolving heart failure symptoms and transfusion-dependent haemolytic anaemia.
BACKGROUND Paravalvular leak (PVL) following surgical aortic valve replacement is associated with significant morbidity, including heart failure and transfusion-dependent haemolytic anaemia. Transcatheter device closure, in contrast to surgical treatment, is a well-recognized treatment option due to its safer complication and mortality profiles. However, regional data and local experience showed that most PVL closures were done using non-dedicated devices which are not purposefully engineered to meet its specific pathology and anatomical complexity. OBJECTIVE To evaluate the feasibility and demonstrate procedural outcome using the Occlutech PLD Occluder device, a dedicated PVL closure device. CASE SUMMARY A 52-year-old man with prior Stanford type A aortic dissection repair, and redo aortic arch replacement with bioprosthetic aortic valve replacement, presented with progressive dyspnoea and transfusion-dependent haemolytic anaemia. Echocardiography confirmed the presence of PVL which was successfully closed with transcatheter strategy, using an Occlutech PLD 61PLD8W Occluder. RESULTS The Occlutech PLD Occluder device was successfully deployed with satisfactory results leading to significant improvement in patient’s symptoms. CONCLUSION The usage of a dedicated PVL closure device yielded positive outcome in our early experience, and supports broader availability and further evaluation of dedicated PVL closure devices in this setting.

Ng et al. (2026) conducted a case report in Aortic bioprosthetic paravalvular leak with heart failure and transfusion-dependent haemolytic anaemia (n=1). Transcatheter paravalvular leak closure using an 8 x 4 mm wide-waisted Occlutech Paravalvular Leak Device (61PLD8W) was evaluated on Procedural success, symptom resolution, and improvement in haemolytic anaemia at 6 to 8 months. Transcatheter Occlutech PLD closure of an aortic paravalvular leak resolved symptoms over 8 months, reduced LDH from 1520 to 743 U/L, and increased haemoglobin from 8.9 to 10.0 g/dL at 6 months.
