Iron deficiency is linked to ~3 ml/kg/min lower peak VO2 in LVAD recipients univariably.

Is iron deficiency associated with reduced peak oxygen uptake (peak VO2) in ambulatory HeartMate 3 left ventricular assist device recipients?

Is iron deficiency associated with reduced peak oxygen uptake (peak VO2) in ambulatory HeartMate 3 left ventricular assist device recipients?
In ambulatory HeartMate 3 LVAD recipients, iron deficiency is common (35%) and correlates with reduced peak exercise capacity in univariable analysis, though not independently after adjusting for markers of heart failure severity or renal function.

Hypothesis-generating and should not yet change iron repletion strategies in LVAD recipients; leaves open whether iron deficiency independently impairs exercise capacity or requires targeted trial evaluation.

Lastowiecki et al. (2026) conducted an observational in Advanced heart failure with left ventricular assist device (HeartMate 3) (n=31). Iron deficiency (transferrin saturation <20%) vs. No iron deficiency (transferrin saturation ≥20%) was evaluated on Peak oxygen uptake (peak VO2, ml/kg/min) (MD -3.14 ml/kg/min, 95% CI -6.11 to -0.17, p=0.04). Iron deficiency was associated with lower peak VO2 (11.11 vs 14.25 ml/kg/min; β -3.14, P=0.04) in univariable analysis among LVAD recipients, though not significant after multivariable adjustment.
