This case report describes a 2-year-old boy with recurrent oral ulcers over 6 months, requiring up to 2 weeks to heal and reaching 8-10 mm in diameter. After excluding Behçet's disease and other conditions, recurrent aphthous stomatitis was diagnosed. Apremilast was initiated with gradual dose escalation to 10 mg twice daily, leading to a significant reduction in ulcer frequency and severity. This case suggests apremilast may be a viable option for very young children with severe recurrent aphthous stomatitis, though careful dose titration and monitoring are essential.
