In colorectal cancer patients receiving chemotherapy or radiotherapy, higher tumor grades (III and IV) and regional or distant tumor stages are associated with a lower competing risk of cardiovascular death.
Background: We aimed to identify the effects of tumor grade and tumor stage on cardiovascular death (CVD) in patients with colorectal cancer (CRC) undergoing chemotherapy (CT) or radiotherapy (RT).Methods: Data from CRC patients who underwent CT or RT therapy from 2004 to 2016 were enrolled into this research. Fine-Gray competing risk regression was adopted to screen independent CVD predictors. Multiple sensitivity tests were conducted to validate the stability of all statistical outcomes. A nomogram was used to visualize the results. Results: A total of 30,222 patients were included in this study. Tumor grade III [adjusted hazard ratio (HR) =0.633, 95% confidence interval (CI): 0.485–0.826, P=0.001], tumor grade IV (adjusted HR =0.494, 95% CI: 0.296–0.825, P=0.007), distant stage (adjusted HR =0.373, 95% CI: 0.299–0.464, P<0.001), and regional stage (adjusted HR =0.765, 95% CI: 0.648–0.902, P=0.002) were associated with a reduced risk of CVD in CRC patients with CT or RT. The risk stratification was significantly different (P<0.001).Conclusions: Tumor grades III and, grade IV, distant stage and regional stage were associated with reduced CVD risk in CRC patients undergoing CT or RT. The management of CVD risk in CRC patients with CT or RT should focus on the tumor grade and tumor stage.

Ye et al. (2026) conducted a cohort in Colorectal cancer undergoing chemotherapy or radiotherapy (n=30,222). Tumor grade (III/IV) and tumor stage (distant/regional) was evaluated on Cardiovascular death (CVD) (HR 0.373, 95% CI 0.299-0.464, p=<0.001). Higher tumor grades (III HR 0.633; IV HR 0.494) and advanced stages (distant HR 0.373, P<0.001; regional HR 0.765) were associated with reduced cardiovascular death risk in CRC patients on CT/RT.
