Abstract Metastatic adrenocortical cancer presents a rare and challenging malignancy with generally a poor prognosis. Current standard of care, such as chemotherapy and mitotane, offers limited survival benefits for these patients. Immunotherapy is increasingly pivotal in managing solid tumors like renal cell carcinoma, melanoma, and hepatocellular carcinoma. Moreover, the availability of mitotane and its monitoring for therapeutic level and toxicity during and after the systemic chemotherapy is difficult and is currently available only in a few centers. This case report documents a patient with metastatic adrenocortical cancer who showed a remarkable response to immunotherapy with nivolumab and ipilimumab, who progressed after initial surgery, adjuvant radiotherapy, and chemotherapy.
