Review highlights clinical trials improving risk factors in post-stroke care, indicating the role of stroke programs.
In the spring of 2024, we authored an article in Stroke Clinician entitled, “Emerging Post-hospital Models of Care: A Primer for Stroke Centers. We described evolving approaches to post discharge stroke care, predominantly within the first 90 days – with some qualifying for reimbursement. These included nurse navigation, stroke clinics and remote monitoring of home-based physiological parameters. As certification standards evolve and secondary stroke prevention gains attention, stroke programs have a vital role to play in supporting survivors and families to manage risk factors and prevent stroke recurrence. To help inform stroke program leaders, we’ve conducted a review of multiple stroke clinical trials employing interventions for improving modifiable risk factors contributing to secondary strokes. This paper describes several clinical studies on post-stroke care delivery designs and highlights the need to derive from these studies interventions with evidence-based components and core outcome measures for application in stroke programs.
