PT. Trisakti Jaya, established in 2005, produces galvalume in Surabaya and employs around 200 people. The company faces issues with product defects, reducing customer satisfaction and causing losses. This research utilizes FMECA (Failure Mode Effect and Criticality Analysis) and RCA (Root Cause Analysis) to address these defects. FMECA helps quantify defect percentages, while RCA identifies solutions. Findings show tears and dents as the highest defects, peaking in December. Defect data exceeded the Upper Control Limits (UCL) with tears at 2451 (UCL 2470) and dents at 3677 (UCL 3700). Causes include improper machine use and poor warehouse lighting. Recommendations include operator training on machine usage and improving warehouse lighting.
