A primary hemangiopericytoma (HPC) of the lacrimal sac in a 40-year-old patient is reported. HPCs are rare soft tissue tumors derived from the pericapillary pericytes with low malignant potential. Only 10 cases of lacrimal sac HPCs have been previously reported. Here, the authors report an additional case that presented clinically with epiphora and painless mass with no recurrence during 36 months of follow-up. The treatment of choice for this neoplasm with frequently unpredictable behavior is total excision with surrounding tissue and long-term follow-up.
