The goal is to analyze the experience of using vulnerosorption in the treatment of peacetime wounds with the prospect of using it in the treatment of gunshot wounds. Materials and methods. There were 270 patients under observation: primary purulent wounds – 216, traumatic wounds – 54. After surgical treatment, the wounds were washed with a 1-3 % aqueous suspension of amorphous highly dispersed silicon dioxide (AHDSD) and drained. The drug was applied in a layer of 3-7 mm. Dressings were performed daily until the wound was cleaned. The clinical dynamics of the wound process and bacterial seeding of the wound were evaluated. The osmotic and sorption (protein, microorganisms) activity of AHDSD was studied. In the ATO zone, 22 wounded were treated with wound dressings based on AWDK during the first level of care. Results and their discussion. Protein sorption is 320 mg/g, microorganisms – 1010 colony-forming units (CFU). AHDSD is devoid of selective sorption and does not cause the selection of resistant strains. Osmotic activity of AHDSD – (503.0±7.3) %, water absorption – (944.1±15.1) %. In primary purulent wounds, the exudate became more liquid from the second day, the wounds were cleaned in (4.9±0.2) days, granulations appeared in (3.5±0.2) days, the beginning of epithelization in (6.8±0.4) days The need to use antibiotics arose in 47.2 % of patients. During 7 days, the bacterial seeding decreased from (8.4±0.7) (lg CFU/cm2) to (4.8±0.1) (lg CFU/cm2), the protein concentration in the exudate – from (53.1±2.3) g/l to (22.7±1.8) g/l. In case of traumatic wounds, the use of AVDC contributed to the successful completion of primary surgical treatment with the application of primary sutures in 52 of 54 patients. Suppuration occurred in 3 patients (5.8 %). In the case of secondary healing, application of AHDSD made it possible to apply secondary early sutures on the 8-11th day. Wound dressings based on AHDSD were applied to 22 wounded with gunshot wounds at the 1st level of care. Hemostatic action is noted. Therefore, AHDSD meets most of the requirements for drugs for local treatment in the first phase of the wound process. The results of treatment using AHDSD are comparable to the results of treatment by the NPWT method. Conclusion. Vulnerosorption using AHDSD is an effective and promising method of treating wounds at various stages of care.
