Older adults (age ≥65 years) account for nearly half of all hospitalizations in both Europe and the United States (US)1. Hospitalized older adults are uniquely vulnerable to inaccurate creatinine-based estimates of glomerular filtration rate (eGFRcre) due to nonrenal factors such as sarcopenia2 and malnutrition3 that affect creatinine levels. Cystatin C-based estimates (eGFRcys) often yield large and clinically relevant discrepancies from eGFRcre4, but the cause of these discrepancies is unclear.
