Obesity management in hyperthyroid patients is complex, particularly when considering the long-term outcomes of different treatment strategies. This study aims to compare the effectiveness of GLP1 analogues and bariatric surgery in managing obese patients diagnosed with hyperthyroidism. Utilizing the TriNetX health research network, we analyzed the records of 2,755 patients treated with GLP1 analogues (Cohort A) and 1,934 patients who underwent bariatric surgery (Cohort B). The analysis focused on mortality, hospitalization rates, incidence of thyroid storm, and thyroid function tests (T4 and TSH levels). Both cohorts demonstrated a 100% survival rate. Cohort A showed a significantly higher hospitalization rate (29.0%) compared to Cohort B (12.4%). Incidences of thyroid storm were rare in both cohorts, with 18 cases in Cohort A and 10 in Cohort B. Laboratory analysis of T4 and TSH levels showed no significant differences between the groups. Our study indicates that while GLP1 analogues are associated with higher hospitalization rates, the incidence of severe complications like thyroid storm is comparably low in both cohorts. These findings suggest that both treatment strategies can be viable options for managing obesity in hyperthyroid patients, with the choice of treatment being influenced by individual patient characteristics and clinical goals.
